You purchased your dog from either a pet store or breeder
AND
It was more than three months ago and less than two years ago.

By completing the Dog Consumer Information Survey you will help veterinary medicine improve the health of all dogs – even prior to purchase. And you will be entered in a raffle to will a $50 gift certificate for pet supplies!
